Each new platform solves a real problem and adds a new place where the truth might be. Eventually employees spend more effort locating the current state of work than changing it.

Why it happens

Tools are adopted per team and per problem, while the work itself crosses both.

Nobody owns the total information path, so no one is accountable for the search cost it creates.

What it may be costing

• Hours per person per week spent assembling context.
• Decisions made on partial information because the full picture is expensive to gather.
• Duplicate records created because the existing one could not be found.

What I would look at

1. 01Time how long it takes a capable employee to answer a simple status question from scratch.
2. 02Map how many systems that answer required.
3. 03Establish one source of truth for shared work before evaluating any additional platform.
